Connecting from website...

Alright, i know the code to connect from the website,

teamspeak://ip.ip.ip.ip:ipip

but could someone give me some html that i can put to let the user select their username to logon with? I don't want people logging on as just guest1 as default. could someone write a little html to ask them their name and logon with that name? :confused:
