Have you tried to reinstall the TS server??
1. Is your server.dbs file writeable?
2. Are you sure the UDP port isn't in use by another application?

Yes, I did attempt a reinstall and that didnt seem to help. The system is a Dual Xeon 2.8 Ghz w/HTT and 2 GB of ram, this specific daemon has roughly 350 to 400 virtual servers on it. We run servers with more then this many virtual servers without an issue.

We utilize mysql so server.dbs does not exist for our daemons, and the mysql database is writeable.
100% Positive the ports are open on the system for our servers, it just randomly started happening when one of our users restarted their server it would not come back online and when we tried to manually restart the server it wouldn't come up thats when we checked the log and got the above error.
This problem appears to still be an issue just can't seem to find a resolution.
TeamSpeak 2 supports MySQL but the developers advise against the use of this feature. The software isn't designed for MySQL versions > 4.
How many virtual servers are running on your machine?
