Audio feedback

I have a clan mate who has terrible feedback whenever he talks and someone else talks at the same time. Additionally his game sound effects are transmitting and drowning out his voice. I have run through all of the usual sound settings with him and he still has this problem. Any suugestions someone could provide would be greatly appreciated.
