What could be causing this, and how do I fix it.
Also, people on my server do not have Voice rights by default.

Change the server from a public server to a clan server

Yeah, I searched, but it's a hard thing to nail down the right words for.

People in my server still don't have voice status by default...

If want all of them to have voice status just untick the flag for moderated.
