Are you sure that you entered the right query port? Normally the query port is 10011, but here it tries to connect to 9100 which is more likely a virtualserver port
The second problem seems to be caused by a PHP version < 5.3

With this viewer its not that simply possible. If it is an option for you to run the light viewer (which could also be included via iframe, but requires php 5.3 and don't has a installscript with so many configuration abilities), I could make some small changes to it, so icons are not longer shown (should be eaysily done within minutes by overwriting some methods in the Viewer class)
