Complaints being deleted

I have developed a system for my server where all the complaints are listed on a website and I have come across two problems. When the page is refreshed a lot within a short period of time, it becomes briefly unavailable and then, once it comes back, all the complaints on the server have been deleted.

I am working with the PHP Library for Teamspeak 3.

Another issue is the fact that each user can only submit one complaint about another user. Is this supposed to be a feature or is there a way to turn this off?

Ok, now nobody on my server can sent complaints. It says 'Complaint sent.' in the server log but nothing appears in the list.

First thing is most likely some flooding issue.
