Server group is not longer associated with user when they log back in

I have a specific server group, that has no real permissions associated with it but its setup to have an icon for it, a set of captains bars, for what we call our clan's "officers". I have noticed this only with this group. But I've noticed users lose this group with their account. The last few days I have noticed users assigned as captains, which I did myself, so I know they were assigned, log back into the server without their captains icon, and they are not in the officers server group. Any idea what is wrong or how I can make it stick?

check if permission b_group_is_permanent is checked to this server group
if not then you just need to assign this permission to the group
