The icons next to users in TeamSpeak that indicate whether they are talking, muted etc. are not read properly with the VoiceOver screen reader. My suggestion, rather than making these icons separate controls that are visible to VoiceOver, is to change the accessibility label so that it says the contact's name followed by a textual representation of the status: For example, "Simon, constant key". I am thinking of administering a server myself and not being able to tell who is speaking is a major problem, especially since the Windows version has even more accessibility bugs and it's impossible to tell there as well. Unlike iOS though, the bugs on Windows are the fault of QT, and are probably not the easiest thing to fix, so I'm leaving those alone.
